
What is Structured Family Care?
Structured Family Care (SFC) is a model of caregiving. Through benefits under Indiana's Aged and Disabled Medicaid Waiver Program, eligible Care Recipients can identify a personal Caregiver to assist them in meeting their daily care needs. The benefits of this Waiver Program allow qualifying seniors and those with disabilities to remain in the comfort of their own homes while receiving care instead of being institutionalized. The Program also empowers Caregivers by provides financial assistance to them for the completion of their caregiving duties. The program also helps maintain the dignity of participating Care Recipients and provides Supportive Services to both the Caregivers and Care Recipients throughout their participating program enrollment.
Our approach
Your Care Connection is a licensed Structured Family Care overseeing agency. We are committed to providing high-quality structured family care services throughout the State of Indiana. After appropriate eligibility is determined by your local Area Agency on Aging, our agency staff works closely with each Client and Caregiver to create a customized service plan that meets their unique needs.
Eligibility Requirements
To be eligible, a Care Recipient must (1) qualify for services under Indiana's Structured Family Care Medicaid Waiver Program, (2) be a resident of Indiana , and (3) meet the appropriate nursing level of care.
An eligible Caregiver must (1) be at least 18 years old, (2) live with the eligible Care Recipient, and (3) be both physically and mentally able to provide the determined level of needed care. Caregivers must also be able to pass a Criminal Background check.
Once eligibility is determined, a Care Recipient and Caregiver are matched with a Structured Family Care overseeing agency, such as Your Care Connection. Our case management and nursing teams at Your Care Connection will provide clients matched with us with the necessary training to maintain compliance with required State and Regulatory Agency standards. As an overseeing agency, Your Care Connection is also responsible for managing the per diem stipend paid to Caregivers.

Here’s How it Works
1. Get Assessed
A visit will be arranged to determine if you’re eligible for Medicaid-funded in-home care. If eligible, you’ll next find out the appropriate level of need for structured family care services with our agency.
2. Choose a Caregiver
You choose a caregiver: your daughter, son, relative, or friend. We come to your home to enroll you in the program.
3. You Get Care – They Get Paid
You get peace of mind by being able to get ongoing care from a person who loves you. They get peace of mind by getting coaching and support to aid in caring for you. Plus, they get paid for their effort!
Choose a family member or trusted friend as your caregiver—and they can earn money while caring for you!
